Bequest Form

Please take your time reading the NUIG Donor Programme Information Leaflet before completing the NUIG Donor Programme Bequest Form. 

Two copies of the Bequest Form must be completed; one to be retained for your files and the other to be sent to Anatomy.  Bequests are finalised following receipt of a letter from Anatomy confirming the registration.  

Only those persons who are able to give their own informed consent and provide their own original signature, may register to bequest and donate their body.

Important Notes 

The time lapse from death to arrival in NUI Galway must be no more than 36 hours. 

Anatomy cannot accept body donations if organs are planned to be donated.
